Documentation that matches how geriatricians plan care
Geriatric practice is care planning as much as diagnosis: chronic disease reviews, team care arrangements, and family discussions that end in a written plan. Medical Scribe records the consultation — in clinic, via telehealth, or during a facility visit — and drafts the structured documentation from it, so the plan is built while you’re still in the conversation.
From medical background to a problem-by-problem plan
Notes can follow the built-in GP MP/TCA template: the patient’s medical background and existing care plan details, the interval clinical history, and a GP Management Plan pairing each problem or need with its goal, treatments, and services — with additional conditions each given their own section. A Mental Health Care Plan template is also available for geriatricians.
Plans the whole team can act on
Your documentation directs physiotherapists, nurses, and the GP for the next six months. Because Medical Scribe records only what was actually agreed in the consultation — the goals set, the referrals made, the review dates chosen — the plan the team receives is the plan the patient heard. You review and sign it first.
